Program Mission:
It is the mission of CM Community Services, Inc. Case Management Program to assist and work closely with individuals who exhibit serious impairment due to mental illness, their families and significant others; and to ensure that they have access to the full array of needed and appropriate services and resources available in the community.
Philosophy:
It is our philosophy that developing and maintaining an individual’s independence will facilitate adjustment and integration within their living environment and community. In keeping with this philosophy we empower recipients and their family members/guardian to make decisions about their care, which we believe will produces better outcomes and an increased quality of life.
Program Description:
The purpose of mental health targeted case management services is to assist individuals (recipients) in gaining access to needed medical, social, educational, and other services.
The primary goal of mental health targeted case management is to optimize the functioning of recipients who have complex needs by coordinating the provision of quality treatment and support services in the most efficient and effective manner.
CM Community Services, Inc. Case Management Program is provided in a safe setting. A wide range of services are organized within a comprehensive case management environment that includes screening and assessment, diagnostic determination, home visits, service care planning, linkage to appropriate services, advocacy, referral to community resources and monitoring of service delivery. Recipients are assigned to case managers that are supervised by a qualified and experienced Case Manager Supervisor(s) who as well may participate in the assessment process and in individual planning and care.
Program Goals:
The overall goal of CM Community Services, Inc. Case Management Program is to increase the quality of life through the provision of targeted case management services. Specific areas of focus may include:
- To eliminate the stigma associated with mental impairment – recovery: Stress is placed on the concept of recovery by assisting the recipient to develop a new meaning and purpose in life. Our goal is for them to live a satisfying, hopeful, and contributing life.
- To empower and meet the needs of priority recipients.
- To build and strengthen community support systems for recipients. Relationships: Develop & support systems where recipients can share with each other and gain peer and community support
- To enhance existing alcohol, drug abuse and mental health treatment by facilitating access to these services through case management.
- To provide a safe and healthy environment to the recipient. – Housing
- To enhance Spirituality (when applicable): Assist / link recipient to community services with the intent to develop or strengthen this area of support which is a strong force within the concept of recovery.
Program Objectives:
- CM Community Services, Inc.’s Case Management Program seeks to achieve the following specific objectives:
- To assist in improving mood and affect in daily living by monitoring referrals to appropriate treatment.
- To assist in providing a safe and nurturing environment through linkage, brokerage, monitoring and referral.
- To assist in improving social, and family supports through integration, linkage, brokerage, monitoring and referral.
- To assist in reducing the need for a higher level of care linkage, brokerage, monitoring, and referral.
Program Services/Continuum of Care:
CM Community Services, Inc.’s Case Management Program provides a continuum of care that ensures all recipients and family members referred are evaluated and provided services and/or referred to primary and secondary service providers to meet their individual needs. The specific areas that demonstrate a continuum of care are as follows:
Screening and Intake Assessment: A screening and intake process is completed within 10 business days of the initial visit. The strengths, needs, abilities, and preferences of each recipient are identified explicitly for integration within an individual plan of care.
Comprehensive Individual Planning: An individual plan is developed with each recipient within the first 30 days of service. The recipient is a full participant in the process and goals and objectives are based on the individual needs, strengths, abilities and preferences. Problem statements are stated in the person’s served own language, which is used to develop service goals. The recipient has open access to the individual plan upon request.
Progress Reviews and Individual Plan Reformulation: Regular scheduled meetings are held with each recipient to review the progress of their individual goals and update and/or revise the Individual Plan. Staff is additionally available to participate in meetings with the recipient and other care providers such as child advocates, guardians’ ad litem, and physicians.
Discharge Planning and Criteria: Discharge planning occurs throughout all phases of the program. The recipient has met discharge criteria when the goals of the individual plan are achieved and referral to support services is completed, when appropriate
Mechanisms to Address the Needs of Special Populations:
CM Community Services, Inc.’s Case Management Program may address the special needs of the recipient through the development and ongoing monitoring and modification of the individual plan. Through this process, the strengths, abilities, needs, preferences and desired outcomes will be developed based on the unique qualities of the recipient. Specific accommodations to address special needs may include the following amplification devices and writing boards for use with the hearing impaired, magnification sheets for the visually impaired, and language interpretation for non-English speaking persons.
